RE: COVID-19 TESTING BY APPOINTMENT

With your help, we can help limit the spread of the disease by following company, local, state and federal guidelines for COVID-19. This includes maintaining at least six feet of distance from people you do not live with, wearing face coverings, and getting tested for the virus if you have symptoms or have been in close contact with someone with COVID- 19.

The spread of the pandemic is expected to spike following the holidays and as such all NROTB employees should remain vigilant and on guard.

We have been in contact with the below local provider that has offered to provide NROTB employees with COVID-19 testing on a by appointment basis:

The completion of a New Patient Package will expedite your processing. We can email the Package to you upon request.

 • People with COVID-19 symptoms

• People notified by a contact tracer or other public health authority that they have

potentially been exposed to someone with COVID-19
